Put your name on it at the top.
Put the due date
Put CSc 4210/6210 on it
Put what it is, e.g. "Quiz 1 rework"

Make what you are doing very very clear to the grader. 

Hand write your solutions - though it is OK to handwrite on a tablet if
   it does not automatically convert your handwriting

Turn it in with the original quiz papers

If you do not have your quiz papers, get with someone in class and
copy the questions, and turn in your solution along with the questions.

In that case, also put a note explaining that you lost your quiz.

If you lost your original quiz, you will need to re-work all problems on it
since you do not have a record of which questions you got correct.

The work will be evaluated, and you can get up to half of the points that
you lost. e.g. if you scored 10/20 and turn in a perfect reworked answer,
your score would go to 15/20. If you score 8/10 on a question, and you 
re-work it, you could earn a new score of 9/10 for that question.

If you got full points on a question, you do not need to re-work it.

You may be asked to explain your solutions.
